They did surgery on a grape. In 2010, Edward Hospital made a demonstration on how precise their surgical system is by peeling a grape with it. Their system was the Da Vinci System. The hospital posted a video of the demonstration on Youtube. 
A business and tech site, Cheddar, posted an edited version of the video and added the words “They did surgery on a grape”. Even though the video was made in 2010, the meme started in 2018.
